Respiratory function, autonomic dysfunction, and systemic inflammation are closely linked in patients with COPD and tidal flow limitation: An exploratory study

چکیده
• In stable COPD there is an ANS activity imbalance, with cardiac sympathetic predominance. It related to airflow obstruction severity, the onset of pulmonary hyperinflation, and emphysematous disease’s prevalence. A relationship between autonomic dysfunction systemic inflammation has been observed. Autonomic low-grade are associated resting tidal expiratory flow limitation. The study aimed investigate interplay among respiratory function, dysfunction, in patients. 19 patients, functional parameters, heart rate variability (HRV), plasma high-sensitivity-C-reactive-protein (hs-CRP) were assessed. Forced oscillation technique (FOT) was used detect absence (NFL) or presence (FL) Subsequently, patients underwent incremental shuttle walking test (ISWT). Twenty healthy subjects also shown as controls. FEV 1 , DL CO lung volumes displayed significant correlations LH/FH ratio (0.56 < r 2 <0.27,p 0.01). found IC/TLC ratio% (r = 0.29,p 0.05) hs-CRP 0.26,p 0.05). Patients FL had greater levels (p 0.05), lower IC/TLC% higher (p<0.001). Worse a LH/HF ratio, directly related, indices dynamic hyperinflation. hyperinflation strong driver dysfunction.

Clinical characteristics of COPD patients with tidal expiratory flow limitation
We have used impulse oscillometry to identify COPD patients with tidal expiratory flow limitation (EFL), which is a measurement related to small airway disease. We report that 37.4% of COPD patients had EFL; these patients had multiple clinical characteristics of more severe disease including lower forced expiratory volume in 1 second values, greater hyperinflation, reduced exercise performance...

Orthopnea and tidal expiratory flow limitation in patients with stable COPD.
BACKGROUND Orthopnea is a common feature in COPD patients, although its nature is poorly understood. OBJECTIVE To study the role of tidal expiratory flow limitation (FL) in the genesis of orthopnea in patients with stable COPD. MEASUREMENTS Tidal FL was assessed in 117 ambulatory COPD patients in sitting and supine positions using the negative expiratory pressure method. The presence or abs...
متن کاملBreathing pattern and gas exchange at peak exercise in COPD patients with and without tidal flow limitation at rest.
Expiratory flow limitation (FL) at rest is frequently present in chronic obstructive pulmonary disease (COPD) patients. It promotes dynamic hyperinflation with a consequent decrease in inspiratory capacity (IC). Since in COPD resting IC is strongly correlated with exercise tolerance, this study hypothesized that this is due to limitation of the maximal tidal volume (VT,max) during exercise by t...
متن کاملMethods for Assessing Expiratory Flow Limitation during Tidal Breathing in COPD Patients
Patients with severe COPD often exhale along the same flow-volume curve during quite breathing as during forced expiratory vital capacity manoeuvre, and this has been taken as indicating expiratory flow limitation at rest (EFL(T)). Therefore, EFL(T), namely, attainment of maximal expiratory flow during tidal expiration, occurs when an increase in transpulmonary pressure causes no increase in ex...
